What moved the curve was rarely another procedure — it was leaders who read risk in the moment, had the right conversation, and made ownership personal. This is what the leadership program behind that shift did, how it was designed, and what it had to survive.

Forty per million hours.
Twelve thousand people in heavy industry across twenty countries, and a lost-time injury rate of 40. What followed took eighteen years — and it wasn't one initiative. It was a change in how leaders led.
Systems got the first drop.
Standards, procedures, supervision — the dependent stage of the Bradley curve. Real progress. Then it flattens: compliance only takes a curve so far.
Leaders made it personal.
The next fall came from leadership behaviour — assessing risk in the moment, having the right conversation, making ownership personal.
Teams made it stick.
The next reduction is interdependent. Safety behaviour becomes a group dynamic — and the standard holds when no leader is in the room. At this stage safety is owned by everyone.

Where the practice was formed.
Dave Noble's practice was formed inside BHP Steel — later BlueScope — a business operating across Indonesia, Thailand, Malaysia, Singapore, Vietnam, China, the United States, Australia and New Zealand. The work was strategy execution, leadership capability, succession, and the design of performance interventions that had to hold up on a plant floor, not just in a boardroom.
He was part of the leadership work behind one of the most sustained safety turnarounds on record. The program described below was, in the words of its own design notes, one of the major components that moved the business from an LTIFR of 40 in 1994, to 3.4 in 2000, to 0.8 in 2012 — across employees and contractors, in every country the business operated in.
The same work later went to Holcim, Cement Australia (a 75% reduction in LTIFR and 87% in TRIFR), Fresenius Medical Care, Select Harvests, Swire Shipping and Butler Manufacturing — and it is the design the Safety Leadership Program is built on.
Four operating principles that made it a leadership program, not a course.
Owned and led by line management
The intervention was the line's, not the safety department's. Leaders ran it, reported on it and were measured by it — which is what made it a leadership program rather than a training event.
Leadership as a critical control
The premise: an incident is a control failure, and leadership behaviour is one of the controls. Felt presence in the field, the pre-shift discipline, the hard conversation — treated as load-bearing, not soft.
Felt leadership and tough love
Participants built the skills to be visibly present and personally demanding at the same time — the combination that makes people care about preventing injury rather than complying with a procedure.
Mixed levels, departments and partners
Cohorts deliberately mixed levels, functions and business partners, so the working relationships and shared style that safety depends on were built in the room, not assumed afterwards.

The room was never the point. The design put the learning where the risk was — on site, over months, with the line manager in the loop.
